Abstract The Italian society of colo-rectal surgery (SICCR) is dedicated to improving the study, prevention and management of the diseases of the colon, rectum and anus. One of the aims of the society is to establish guidelines to the treatment of these diseases. These guidelines are based on the international literature and on the best available evidence. Clinical practice guidelines are one of the most important instruments to provide therapeutic decision-making support, based on the best scientific evidence available at the time. Guidelines are advisory and not prescriptive, susceptible to continual variations secondary to innovations and new scientific evidence. These guidelines are a guide for all colo-rectal surgeons and physicians who approach anal cancer.

Treatment of Crohn’s disease by means of surgery has been chosen by the ACOI, committee for guidelines, under the strict supervision of Giovanni Romano and other members (Many of whom are also members of SICCR) as a topic which is continuously evolving and that engages a great percentage of surgeons even in medical centres that are not mentioned.
The objective of this work is to provide the members of ACOI through the opinion of experts and the close examination of evidence regarding its treatment in the multiple symptoms of this disease, and a surgical strategy based on evidence based medicine (EBM) as a useful guide to its best practice.
Such a constructive task can be used by all surgeons, as a principle means based on the scientific evidence available at present, modelled and adapted to the cultural and instrumental circumstances of its own coloproctology unit.

Nei paesi occidentali il cancro del colon retto (CCR) rappresenta il terzo tumore maligno per incidenza e mortalità. E`il cancro più comune del tratto gastro-intestinale, la cui incidenza stimata in Italia nel 2009 è di 69 nuovi casi su 100.000 abitanti per i maschi e 38 su 100.000 abitanti per le femmine. La mortalità è di 21/100.000 e 11/100.000 abitanti nei maschi e nelle femmine, rispettivamente. La malattia è più frequente a partire dai 60 anni, raggiunge il suo picco verso gli 80 anni colpendo in egual misura uomini e donne, il 6-8% dei casi viene però diagnosticato in pazienti con meno di 40 anni. Al momento della diagnosi il 10% dei pazienti ha un tumore in situ, il 30% ha diffusione locale, il 30% regionale e un altro 30% ha un tumore metastatico.
Negli ultimi anni si è assistito ad un aumento delle diagnosi di CCR, per un miglioramento dell’iter diagnostico, ma ad una diminuzione della mortalità, attribuibile soprattutto alla diagnosi precoce (programmi di screening) e al miglioramento della terapia.
Nonostante tali progressi la sopravvivenza a 5 anni rimane complessivamente all’incirca del 40-60 % (90% per i tumori con diffusione locale, 60% a distanza, 5% per i cancri metastatici).

L’Associazione Italiana Gastroenterologi ed Endoscopisti Digestivi Ospedalieri (AIGO) ha lo scopo di sviluppare la conoscenza delle patologie digestive, promuovere il progresso nella prevenzione-diagnosi-cura e riabilitazione delle malattie gastroenterologiche, e sviluppare la ricerca.
La Società Italiana di Chirurgia Colo-Rettale (SICCR) ha lo scopo di assicurare il più alto standard terapeutico mediante la valutazione e l’introduzione nella pratica medica dei progressi della scienza nei campi della prevenzione-diagnosi e cura delle malattie del colon, del retto e dell’ano.
La Commissione congiunta AIGO/SICCR è stata composta con membri delle due società scientifiche scelti per la loro esperienza nelle problematiche funzionali ed organiche colorettali.
L’obiettivo della Commissione è stato di elaborare un ”Consensus Statement” sugli aspetti diagnostici e terapeutici della stipsi funzionale e della defecazione ostruita attenendosi ai livelli di evidenza, attualmente disponibili in letteratura, da cui ricavare un grado di raccomandazione.

In the development of clinical guidelines for the treatment of colon cancer with a minimally invasive technique, we referred to the EAES Consensus Conference held in Lisbon in 2002 [1]. We searched PubMed and Medline from 1990 to 2006 using the keywords “laparoscopy” and “colorectal neoplasms”; embedded references also were reviewed. Surgical techniques are difficult to evaluate scientifically, so the level of evidence of articles on this topic is, regrettably, low.

The choice of the best surgical technique must always be based not only on the surgeon’s personal experience but above all on the scientific evidence supporting the superiority of one surgical technique compared with another. The results of surgical treatment of haemorrhoids can be assessed on the basis of its effect on various parameters, namely postoperative pain, time taken to return to normal working and relational activities, hospital stay, the incidence of early and late complications, aesthetic and functional results, the incidence of recurrence and finally, its cost. Thus, any comparison of different techniques must take into account all these factors.
